Inspector Notes

An unannounced complaint investigation was conducted with the Assistant Director on 11/22/22 from 1:10pm to 2:30pm in response to a complaint received in the licensing office regarding allegations related to ratios, accident/incident reporting, and toileting procedures. Based on review of documentation and interviews conducted, the allegations were unable to be substantiated due to insufficient corroborating evidence. There were no violations cited.
