Inspector Notes
An unannounced monitoring inspection was initiated on 06/30/25 and completed on 07/10/25 in response to a self-report received by the licensing office on 06/27/25 relating to supervision. The inspector reviewed two children?s records and one staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
