During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on August 27, 2025, and completed on August 27, 2025, in response to a complaint received by the licensing office on August 12, 2025 relating to allegations of preventing the spread of disease and toileting procedures. The inspector reviewed one child?s records and two staff records on-site. This inspection included document review, a tour of the facility, and interviews.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to Immunizations for children and are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The record of child #1 contained documentation of immunizations dated 31 days after enrollment.
Plan of Correction: Per the director: Going forward, we will obtain documentation of immunizations by the first day of attendance.
