During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was completed on 12/10/2024. The on-site inspection began at 10:30 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 17 pts. The inspector reviewed 4 children?s records and 3 caregiver/household member(s) records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/18/2024. A POC submitted after this date will not appear on the public website.

A person 14 years of age and older who is living in the home must have a search of the
central registry.

Household member #1 is over 14 years old and did not have a search of the central registry on record.
Plan of Correction: Make sure that one household member has central registry on record.
Standard 8VAC20-800-120-A
The provider shall maintain a record for each caregiver.

Caregiver #2 did not have a record available.
Plan of Correction: Before caregiver #2 begins working. Must have a record of her.
Standard 8VAC20-800-210-A
Caregivers shall obtain a minimum of sixteen clock hours of training annually.

Caregiver #1 did not have any annual training hours documented.
Caregiver #2 had 12.5 annual training hours documented.
Plan of Correction: Keep that document on record at all times.

Infant feeding or activity tables, shall be used according to the manufacturer's instructions. When occupied by a child, a safety strap shall be used and securely fastened.

Two children were observed sitting in an infant feeding table without straps on.
Plan of Correction: Each time that I use the feeding tables I must strap on the children when they sit down.

Child #2 was enrolled for 8 days and did not have a complete record available.
