During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site focused inspection was completed on 03/20/2025, as a part of the provisional licensure period. The on-site inspection began at 10:30 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 13. The inspector reviewed 6 children?s records and 2 caregiver records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 04/02/2025. A POC submitted after this date will not appear on the public website
Violations
14The copy of the provisional license was posted behind the door to the family day home. When the door is open the license is blocked from view.
Caregiver #2, who had been working with the children for one month, did not have a completed sworn disclosure statement.
Caregiver #2, who had been working with children for one month, did not have a completed national criminal background check.
Caregiver #2, who has been working with children for 1 month, did not have a central registry search requested.
The provider shall maintain a record for each caregiver.
Caregiver #2 did not have a record for review.
There was no tuberculosis screening for Caregiver #2, who has been working with children for one month.
Caregiver #2 has been working with the children for 1 month and did not orientation training.
Disinfectant wipes were on a lower table by the bathroom within reach of children.
Caregiver #1 mislead inspectors by not providing factual details to specific questions that were asked about caregiver #2 and additional adults in the home.
The supplemental page from the inspection on 12/13/2024 was posted in the family day home reveling confidential information.
Caregiver #1 did not wash their hands with liquid soap and warm running water after diapering child #5.
Child #5 did not have their hands washed with liquid soap and warm running water after their diaper change.
The diapering surface was not cleaned and sanitized after the diaper change of child #5.
The menu posted ended on 3/17/2025. The remainder of the week was not posted.
