Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.
An investigation, regarding a self-reported incident, was initiated on 03/07/2022 and concluded on 03/10/2022. An in-person inspection was conducted. The physical plant, documentation and video were reviewed. Interviews were conducted. The inspector reviewed compliance in the areas of 8VAC20-780-340, 8VAC20-780-370, 8VAC20-780-440, 8VAC20-780-510, 8VAC20-780-550 and 8VAC20-780-560.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interviews, document review and video review, non-prescription medication was given without written authorization from the parent. Evidence: A non-prescription lotion, not listed on the Written Authorization Form from the parents, was rubbed on the chest of Child A.
Plan of Correction: Any unlabeled lotion was moved from class. Staff meeting was held to discuss the importance of authorizations on any non-prescription meds.
