During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 4/30/2025 and completed on 4/30/2025, as a part of the licensure period. The on-site inspection began at 9:45am and ended at 12:19pm. The inspector reviewed compliance in the areas listed above. There were 105 children present and 19 staff. The inspector reviewed ten children?s records and ten staff records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/6/25. A POC submitted after this date will not appear on the public website.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

The center did not request a criminal history, child abuse and neglect registry, and sex offender registry search from one state for Staff #9, who has been employed for three months and was working with children at the time of the inspection.
Plan of Correction: Sex offender search results were sent (emailed) on 4/30/2025. We have reached out to CA dept of social services and ask the staff member to fill out and notarize form so we can mail it out.

Physical examinations completed by a physician must be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of child #3, who had been enrolled for six months.
Plan of Correction: Parents of child was contacted on 5/1/25 and physical will be brough in after child's next visit to pediatrician which is in few days.

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.

Staff #8, who is working with children, has not yet completed this course which is overdue by two years and three months.
Plan of Correction: Staff #8 has started to take the course. We will make sure each new/old staff member will have this certification.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

In the two year old, caterpillar classroom, a can of disinfectant was stored on the diaper changing table, within reach of children.
Plan of Correction: As this was mentioned, I went to class and spoke with staff to ensure that all substances were out of reach and locked. Staff meeting was conducted on 5/2/25.

Cribs when being used by infants must only have a bottom cover.

Child #5 was sleeping in the crib with a loose blanket in addition to the bottom cover.
Plan of Correction: The blanket was removed. Staff meeting was conducted as the end of day on 4/30/2025 and showed a video called "why its dangerous to have a blanket in a baby's crib."

The diapering area must have a nonabsorbent surface for diapering or changing.

The diaper changing pad in the Ducklings classroom had approximately three areas that were torn.
Plan of Correction: A new pad was ordered and the ripped pad will be replaced. Teacher also use a 12 x 16 sheet under each child being changed.

Over-the-counter skin products must not be kept or used beyond the expiration date of the product.

Four diaper creams were expired: Child 2's expired by ten months, Child 3's by fourteen months, Child 4's by nine months, and Child 6's by one month.
Plan of Correction: Parents were notified and they filled out new forms and new products were brought in 5/1/25. We will make sure to go overs checklists periodically to ensure files are complete.
