The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
The center did not request a search of the child abuse and neglect registry from one state for Staff #3, who has been employed for more than five months and was working with children at the time of the inspection.

The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children.
Staff #3 is currently working and has a TB screening that was dated 29 days after their first day of employment.

In the infant classroom, ladybugs, and the older infant classroom, ducklings, the cleaning solutions and sanitizing solutions were stored on top of the sink, out of reach of children.
