Inspector Notes

A religious exempt renewal inspection was conducted on April 25, 2022. There were 30 children present, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant and backgrounds. A total of 8 staff records were reviewed. Based on review of 8 staff records, 8 did not contain the required documentation of completed background searches.
Evidence
The record for staff #1, #2, #4, #5, #6, #7 and #8 did not have documentation of a central registry background check within 30 days of employment.
The record for staff #3, #4, #6 and #7 did not have documentation of a signed sworn statement before the first day of employment.
Plan of Correction: I did not have my computer today, some the central registry searches may have already been completed. I will check and make sure the documentation is in their record. If the central registry hasn't been completed, it will be submitted and the letter will placed in their record.
I will have the staff sign the sworn statement and place it in their record.
All sworn statements will be singed before their first day of employment.
