During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 7/16/25 and completed on 7/21/25, as a part of the licensure period. The on-site inspection began at 10:57 AM and ended at 12:35mPM. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers; the point total was18. The inspector reviewed 5 children?s records and 2 caregiver records on-site.
This inspection included; document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-120-B
Assistants' and substitute providers' records shall contain the following: two references prior to employment, emergency contact information, employment date.
Caregiver B 's record did not contain documentation of references, employment date and an emergency contact.
Plan of Correction: The caregiver record will be updated to include the missing information.
Standard 8VAC20-800-60-B
A child's record shall contain parent employment information.
Child #2's record did not contain the name, address or phone number for the employer of the parents.
Plan of Correction: Parents will be contacted to provide the missing information.
Standard 8VAC20-800-70-B
The provider shall obtain the parent's written acknowledgement of the receipt of the required policies and procedures.
Children #1 and 3's records did not contain written acknowledgment of the Provision of the Emergency Preparedness Plan.
Children #2 and 4's records did not contain written acknowledgment of the Declaration of Insurance.
Child #5's record did not contain written acknowledgment of the Decision to administer medication policy. (Incomplete)
Plan of Correction: Parents will be provided the policies in writing for signatures.

The family day home shall obtain documentation of additional immunizations for a child who is not exempt from the immunization, every six months until the age of two.
Child #5's updated immunization record was overdue by 5 months.
Plan of Correction: The parent will be asked to provide an updated immunization record.

When meals or snacks are provided by the family day home, the menu for the current one-week period shall be dated.
