Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 8/29/24 and completed on 8/29/24. The on-site inspection began at 10:10am and ended at 11:45am. The inspector reviewed compliance in the areas listed above. There were 9 children present and 3 staff. The inspector reviewed 5 children?s records and 5 staff records on site. This inspection included document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-150-B
Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.
The immunization document for children #2, #3, and #5 were not signed and dated as required.
The children are newly enrolled and have been in attendance.
Plan of Correction: Immunizations will be signed and dated for all children as required.
Standard 8VAC20-780-160-A
Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening at the time of employment and before coming into contact with children.
Staff #1 and #4 had a tuberculosis document that was dated 3-4 months after hire. The staff have been actively working with the children since hire.
Plan of Correction: Tuberculosis screenings will be obtained at the time of hire as required.
Standard 8VAC20-780-240-A
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.
Staff #1 has not completed the orientation course as required, and has been employed and working with the children over 90 days.
Plan of Correction: The staff will complete the orientation course within 90 days as required, or they will not be allowed to work.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain emergency contact information and date of attendance.
The records for children #1-#3 and #5 did not have a second emergency contact listed as required.
The records for children #1-#5 did not have the date of attendance listed as required.
Children #2, #3, and #5 are newly enrolled; child #1 and #4 have been enrolled over a year.
Plan of Correction: The forms will be updated to ensure that a second emergency contact is listed with name, address, and phone number as required. Parents of all children will complete the new form for full compliance.
