Inspector Notes
An unannounced monitoring inspection was conducted today between the hours of approximately 9:40 a.m. and 10:40 a.m. There were 3 children present with the Provider and 1 assistant for a total of 4 points within the supervision guidelines. The children were observed watching television and playing with toys. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children, caregiver and household member records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
7Evidence: 1. The family day home was not able to produce a carbon monoxide detector during the inspection.
2. The provider stated that they do not have a carbon monoxide detector.
Evidence:
1. There was chipped paint on the TV stand in the living room.
2. There was torn fabric on the sofa in the living room.
Evidence:
1. Potentially poisonous substances including, one bottle of acne medication, one air freshener spray and one bottle of baby oil were located on the bathroom sink, accessible to children.
2. Potentially poisonous substances including one toilet bowl cleaner, three bathroom cleaner sprays, one laundry detergent, and one disinfectant spray were located in an unlocked cabinet under the bathroom sink, accessible to children.
3. A bottle of hair removal cream was located in an unlocked drawer at the bottom of the bathroom sink, accessible to children.
4. Laundry detergent was located on the bathroom floor, between the toilet and sink, accessible to children.
Evidence: The last day the family day home recorded attendance was 01/12/2022.
Evidence: The parent?s signed acknowledgement of the review of the emergency contact information for Child #1 was dated 08/01/2020.
Evidence: The provider did not obtain verification of identity and age for Child #2.
Evidence: The most recent emergency plan review was dated 10/31/2020.
