Inspector Notes

An unannounced, on-site monitoring inspection was conducted 3/17/2025 from 11:00am - 12:30pm. There was 1 child and 1 caregiver present during the time of the inspection; totaling 2 points. The inspector reviewed 3 child records, 4 caregiver/household records onsite. The
inspection included document review, interviews, observations, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on March 26, 2025. A POC submitted after this date will not appear on the public website.

A physical examination conducted prior to attendance shall have been within twenty-four months prior to attendance for children six years of age and above.

REPEAT VIOLATION
Potentially poisonous substances shall be stored away from food and in areas inaccessible to children.

Two large jugs of laundry detergent was found in the kitchen area on the floor underneath the microwave and near containers of food. The kitchen has a gate that was not closed, making the detergent accessible to children.
