During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was conducted 3/17/2025 from 11:00am - 12:30pm. There was 1 child and 1 caregiver present during the time of the inspection; totaling 2 points. The inspector reviewed 3 child records, 4 caregiver/household records onsite. The
inspection included document review, interviews, observations, and a tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on March 26, 2025. A POC submitted after this date will not appear on the public website.
Violations
6The physical examination for Child #1 was completed seven years prior to attendance.
The physical examination for Child #2 was completed four years prior to attendance.
Potentially poisonous substances shall be stored away from food and in areas inaccessible to children.
Two large jugs of laundry detergent was found in the kitchen area on the floor underneath the microwave and near containers of food. The kitchen has a gate that was not closed, making the detergent accessible to children.
A child's record shall contain two designated persons to contact in case of an emergency if the parent cannot be reached.
The record for Child #1 and #2 had one emergency contact that was not a parent.
Before the child's first day of attendance, parents shall be provided required information in writing.
The record for Child #1 and #2, enrolled for more than six months, and Child #3, enrolled for one year, did not contain documentation that the policy for administering medication was provided to the parents in writing.
A bottle of cough syrup and vitamins were found unlocked in the open kitchen pantry, making them accessible to children.
The provider did not have documentation of additional immunizations between the fourth and sixth birthdays for Child #1, who is ten years of age.
