Inspector Notes

An unannounced renewal inspection was conducted today from 10:40am-12:30pm. There were 102 children directly supervised by 17 staff. The physical plant, 14 staff records, 7 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, eating lunch, playing outside, and participating in a number activity. Handwashing procedures were observed. There was an assortment of age appropriate materials for the children in care. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Based on record review, each staff member did not submit documentation of a negative Tuberculosis screening at the time of hire.
Evidence:
Staff 13's Tuberculosis screening was conducted sixty nine days after hire.
Plan of Correction: The staff currently has a negative Tuberculosis screening on file.
Standard 8VAC20-780-240-A
Based on record review, staff did not complete the Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.
Evidence:
Staff 13 did not have documentation to certify completion of the Virginia Department of Education-sponsored orientation course. The staff member had been employed since 6/6/2023.
Plan of Correction: THe staff will have completed the Virginia Orientation course by 12/08/2023.

Based on observation, areas in the center were not maintained in a safe and operable condition.
Evidence:
Two cabinets in the Beginning Preschool Room were observed with broken handles.
