Inspector Notes

A complaint investigation was conducted today in response to a complaint received by the Fairfax Licensing Office on January 24, 2024. The complaint alleged areas of concern regarding Tuberculosis screenings, staff annual training, communicable disease reporting, and hand washing.

A review of 2 staff records was conducted, one classroom was observed, and three staff members were interviewed.

Evidence was found to support the allegation of an untimely Tuberculosis screening for staff and therefore the complaint is deemed to be valid. One other non-complaint related violation was cited during todays inspection.

COMPLAINT RELATED

Based on record review, each staff member did not submit documentation of a negative Tuberculosis screening at the time of employment and prior to coming into contact with children.
Evidence:
Staff 1 was hired on 10/24/2023 and did not have the results of a negative Tuberculosis screening on file until 1/17/2024.
Plan of Correction: The staff member was dismissed from active duties on 1/4/2024 until a negative screening was received on 1/17/2024.

Based on record review, staff did not complete the Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.
Evidence:
Staff 1 was hired on 10/24/2023 and does not have documentation on file of having completed the Virginia Department of Education-sponsored orientation course
Plan of Correction: The training will be completed this week.
