Inspector Notes
A complaint investigation was conducted today in response to a complaint received by the Fairfax Licensing Office on January 24, 2024. The complaint alleged areas of concern regarding Tuberculosis screenings, staff annual training, communicable disease reporting, and hand washing.
A review of 2 staff records was conducted, one classroom was observed, and three staff members were interviewed.
Evidence was found to support the allegation of an untimely Tuberculosis screening for staff and therefore the complaint is deemed to be valid. One other non-complaint related violation was cited during todays inspection.

2Based on record review, each staff member did not submit documentation of a negative Tuberculosis screening at the time of employment and prior to coming into contact with children.

Staff 1 was hired on 10/24/2023 and did not have the results of a negative Tuberculosis screening on file until 1/17/2024.
