Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.

A renewal inspection was initiated and concluded on 1/21/2022. The provider was contacted by telephone and a virtual inspection was conducted. There were 7 children present (12points), ranging in ages from 4years to 5years, with the provider and one assistant caregiver supervising. The Inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care, and emergencies. A total of 2 children's records and 3 caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of caregiver health records, it was determined that the provider did not obtain for each caregiver a current Report of Tuberculosis Screening form every two years from the date of the first screening.
Evidence:
The TB screening for caregiver A expired on 1/8/2022.
Plan of Correction: The repeat TB screening will be completed.

Based on a review of children's records, it was determined that one record does not contain all required information.
Evidence:
Child record B does not include documentation of the review of the child's emergency contact information. The parent most recently reviewed this record on 2/28/2020.
Plan of Correction: I will ask the parent to review the information.
