Inspector Notes

An unannounced monitoring inspection was conducted on May 12, 2023 from approximately 10:00 am-11:21 am. Children were observed dancing, morning activitiy and freely playing. Three staff records and three children records were reviewed during the inspection. Licensing Inspector reviewed indoor areas, supervision, activities, equipment, emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and staff interview, the licensee did not ensure that all written records of children injuries had all components required.

Evidence:
