Inspector Notes

An unannounced, on-site monitoring/ inspection was initiated on 5/13/2025 and completed on 6/6/2025. The on-site inspection began at 1:00 pm and ended at 3:00 pm. The inspector reviewed compliance in the areas listed above. There were 35 children present and 6 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-70
The center is required to maintain documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position, and orientation and training as required.
The record for staff #4, who has been in the role of lead teacher for over 5 months, did not contain a transcript or diploma as required by the position.
Plan of Correction: Staff #4 resigned from her position on 5/16/2024. The program director will ensure that a copy of the staff's high school diploma or transcript are obtained and maintained in program leaders' records.
