Inspector Notes
A Subsidy Health and Safety inspection was initiated on 4/17/2023 and concluded on 4/18/2023 to review supplemental health and safety requirements. There were 24 children present with 3 staff supervising.
A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Based on a review of five (5) staff records and interview, the vendor did not ensure that one (1) staff that worked directly with children had within 90 days of the date of employment: current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Evidence:
1. The record of staff #3 (date of employment: 6/9/2022) did not contain record of a CPR certification.
2. Administration acknowledged that there was not record of a certification for staff #3.

Based on a review of five (5) staff records and interview, the vendor did not ensure that one (1) staff that worked directly with children had within 90 days of the date of employment: current certification in first aid appropriate to the ages of children in care.
