Inspector Notes
An unannounced renewal inspection was conducted today between 12:30pm and 1:30pm. There were 2 children present with the Vendor for a total of 7 points within the supervision guidelines. The children were observed resting during today's inspection. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. Violations were found during todays inspection and can be viewed on the Violation Notice.

Based on record review and interview, children?s records did not contain all required information.
Evidence:
1. Child #1?s record did not contain the name, address and telephone number of someone to be contacted if their parents could not be reached, immunization record, and written agreements between the child and the vendor.
2. Child #1 and Child #2?s record did not include proof of the children?s identities. The vendor confirmed that she had not asked for proof of the children?s identities.

Based on observation and interview, pillows were used for children less than two years of age while sleeping.
Evidence: Child #2 (21 months old) was observed sleeping with two fluffy blankets folded underneath their head. The vendor stated that she folds the blankets to make a pillow for the child.
