Inspector Notes
A Subsidy Health and Safety Inspection was conducted on 9/17/2023 from 12:20pm through 1:15pm. There were 3 children directly supervised by the vendor. During the inspection the home, 3 children were observed sleeping, emergency supplies, procedures and drills were inspected. Children's records, vendor information and household member information was reviewed.
Information gathered determined violations with applicable code or requirements and are listed on the Violation Notice.

Based on interview, the vendor did not ensure that each household member undergo background checks in accordance with ? 22.1-289.040 of the Code of Virginia.
Evidence: Household member #1 turned 18 years old in June 2023. The vendor stated that the household member has not yet undergone fingerprinting and has not completed a sworn disclosure statement.

Plan of Correction: My contact with the county is helping me make an appointment. They usually comes to my house to help me because of the children. I will reach out to them.

Based on record review, childrens records did not contain all required information.
Evidence:
1. Child #3s record did not contain the name, address and telephone number of someone to be contacted if their parents could not be reached.
2. Child #2 and Child #3s record did not include proof of the childrens identities.

Based on observation and interview, cribs were not provided for children from birth through 12 months of age.
Evidence: During the inspection, child #1 (5 months) was observed napping on the vendors couch. The vendor stated that the older children use the cribs upstairs.
