Inspector Notes
A Subsidy Health and Safety Inspection was conducted on 8/19/2024 from 10:15am through 11:00am. There were no children present during the inspection. During the inspection the home, emergency supplies, procedures and drills were inspected. Four children's records, vendor information and household member information was reviewed.
Information gathered determined violations with applicable code or requirements and are listed on the Violation Notice.

Based on record review, childrens records did not contain all required information.
Evidence:
1. Child #1s record does not contain the address of an emergency contact.
2. Child #2 and child #3 records did not contain the childrens home address or proof of the childrens identity.

Plan of Correction: I will ask the families for the addresses. The other family is trying to get the birth certificates, but I will ask for them again.
