Inspector Notes
An unannounced monitoring inspection was conducted May 24, 2024, between the hours of 9:28am - 11:05am. There were 7 children (15 points) directly supervised by 1 caregiver. The home, 1 caregiver record, 7 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and building with blocks. There was an assortment of age-appropriate materials for children. Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within five business days in order for it to be included on the public website. 1. The record for caregiver #1 did not contain documentation of a current fingerprint based national criminal history search. The most recent fingerprint based national criminal history search results were dated 08/23/2018.

1. Caregiver #1's record had a tuberculosis screening form dated 11/04/2021.
Based on observation, the provider did not ensure that potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents and disinfectants were stored in areas inaccessible to children.
Evidence:
1. One bottle of cleaning spray was located on the back lower shelf in the bathtub.
Based on observation, child resistant protective covers were not installed on all unused electrical outlets.
Evidence:
1. One unused electrical outlet located in the downstairs kitchen was not covered with a protective cover.
2. One unused electrical outlet located in the childcare bathroom was not covered with a protective cover.
Evidence:
1. Child #5's (date of enrollment: 03/29/2023) and Child #6's (date of enrollment: 03/20/2023) records did not contain documentation of an annual review of the child's emergency contact information.
2. Child #2's record did not contain documentation of one emergency contacts address.
3. Child #7's record did not contain documentation of one parent's phone number.
