Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 8/23/24. The on-site inspection began at 9:50am and ended at 11:40am. The inspector reviewed compliance in the areas listed above. There were 34 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/30/24. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
Based on observation and interviews, it was determined the facility did not ensure all areas and equipment of the center shall be maintained in a safe and operable condition.
Evidence:
1.There was a scooter with a broken wheel on the playground.
a.Staff #5 confirmed it was broken.
Plan of Correction: The center director or designated staff member will examine all toys on the playground and remove any broken objects to ensure the safety of children.
Standard 8VAC20-780-500-A
Based on observation it was determined that the facility did not ensure that children?s hands are washed with soap and running water after toileting, and coming into contact with urine, feces or blood.
Evidence:
1.Child #5 did not have his hands washed after having a diaper change.
a. Immediately upon completion of the diaper change and without washing the child?s hands, the staff member placed child #5 in an excer-saucer.
Plan of Correction: The center director will ensure hand washing after each diaper change. Our policy is to take care of the whole child "faces, hands, clothes and laces". All staff members.
