Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

A monitoring inspection was initiated on 9/2/2021 and concluded on 9/3/2021. The director was contacted by email to initiate the inspection. There were 8 children present and 2 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview it was determined the center did not ensure that each staff member had an updated TB screening at least every two years from the date of the last screening or testing.
Evidence:
The record for staff #1 did not contain current TB screening. The screening available was dated 1/7/19. Staff confirmed that this was the most current TB screening form.
Plan of Correction: Staff #1 will get updated TB screening.
