Inspector Notes

An unannounced monitoring visit and facility tour was conducted from 10:00am- 11:25 am. At the time of inspectors' arrival there were 7 infants in care with 2 staff members and 1 administrator. Children were observed interacting with staff, nap, and diapering/hand washing. A sample of 2 children's records and 2 staff records were reviewed. Injury reports were also reviewed. Director reported that there are no medications being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on record review and interview it was determined the center did not ensure that each staff member submitted to fingerprinting as required.
Evidence:
1. The record for staff #1 did not contain documentation of fingerprinting.
2. Staff confirmed that the documentation was not available during the inspection.
Plan of Correction: Staff #1 will schedule an appointment for fingerprinting.
Standard 8VAC20-780-130-E
Based on record review and interview it was determined the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. The record for child #2(under the age of two) did not contain documentation of updated immunizations. The immunization documentation available was dated 8/28/21.
2. Provider confirmed that this was not available during this inspection.
Plan of Correction: Provider will request this documentation from the parent.
Standard 8VAC20-780-140-A
Based on record review and interview it was determined the center did not ensure each child had a physical examination by or under the direction of a physician within the required time frame.
Evidence:
1. The record for child #1(first day of attendance 6/7/21) did not contain documentation of a physical examination.
2. Provider confirmed that this was not available during this inspection.
Plan of Correction: Provider will request this documentation from the parent.

Based on record review and interview it was determined the center did not ensure that staff resubmit TB test results at least every two years from the date of the first initial screening or testing.
Evidence:
1. The TB documentation for staff #2 was dated 3/11/20.
2. Staff #2 confirmed that she did not have an updated TB test or screening.
Plan of Correction: Staff #2 will get an updated TB test or screening.
