Inspector Notes

An unannounced monitoring visit and facility tour was conducted on 8/5/2022. At the time of inspectors' arrival there were 5 infants in care with 2 staff members. Children were observed interacting with staff, napping, diapering, and hand washing. A sample of 3 children's records and 2 staff records were reviewed. There have been no injuries and medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on record review it was determined the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
Child #1(under 2 years) did not have updated immunization documentation. The documentation in the record was dated 1/10/2022.
Plan of Correction: Staff will request documentation of updated immunizations from parents at pick up.
Standard 8VAC20-780-550-D
Based on record review nd interview it was determined the center did not implement monthly practice evacuation drills.
Evidence:
1. There were no evacuation drills for the months of April, May, June, and July 2022.
2. Staff confirmed they have not practiced an evacuation drill since March 2022.
Plan of Correction: Staff will conduct monthly evacuation drills.

Based on observation it was determined the center did not ensure prepared infant formula was dated and labeled with the child's name.
Evidence:
The prepared infant bottles were labeled with names but did not contain dates.
Plan of Correction: Prepared bottles will be dated.
