Inspector Notes
An announced on-site inspection was initiated on July 12,2024 and completed on July 12, 2024. The on-site inspection began at 10:50am and ended at 11:15am. There were 12 children present and four staff. The inspection included measurements and inspection of an add-on area, per request of the center. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on observation the center did not ensure that areas and equipment of the center are maintained in a safe condition.
Evidence:
1.There was a ceiling tile missing from the ceiling and on the floor of the fellowship hall.
2. A cinder block wall at the back of the fellowship hall had many areas of peeling paint.
