During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 3/18/26. The on-site inspection began at 11:50am and ended at 1:50pm. The inspector reviewed compliance in the areas listed above. There were six children present and four staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations. A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards. Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/1/26. A POC submitted after this date will not appear on the public website.
Violations
12The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The immunization report for Child #2 show that the report was not received before the child attended the center, which is required
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. The center did not obtain additional immunizations in the required six month timeframe for Child #1.
A physical examination was not obtained for Child #2 within 30 days after their first day of attendance, which is required.
A tuberculosis (TB) screening is required every two years from the last screening.
The record for Staff #3 (employed for two years) did not contain a follow-up TB screening, which is required.
Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care. The records for Staff #2, Staff #3, and Staff #4 did not have documentation to show that 16 annual hours of training were completed in 2025.
No child shall be allowed to drink or eat while walking around. A 15 month old infant was observed walking around the classroom while drinking from a cup.
Staff who work directly with children shall annually attend at least 16 hours of training and staff development activities, to include the department's health and safety update course.
The records for Staff #2 and Staff #3 did not contain documentation that the department's health and safety update course was completed in 2025.
