Inspector Notes

A non-mandated monitoring inspection, resulting from an Intensive Plan of Correction, was conducted on 04/12/2023. There were 21 children, ages 2 years- 4 years, 5 staff members providing direct care and supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and training, and background checks. A total of 4 children?s records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 22.1-289.035-A
Based on observation and record review, the center failed to ensure that no person shall be hired for compensated employment, continue to employee, or permit to serve as a volunteer, any person who has been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth as defined by the Code of Virginia.
Evidence: Staff 1 had a documented employment date of 11/17/2022. The most recent finding of the central registry search for Staff 1, dated 02/17/2023, resulted in a ?Match? as the individual has been the subject of a founded complaint of child abuse or neglect within the Commonwealth of Virginia. Staff 1 was observed providing direct care and supervision during the inspection on 4/12/2023.
Plan of Correction: Employment will be terminated, and the portal will be used to ensure central registry checks are received in the required timeframe.
Standard 8VAC20-780-60-A
Repeat Violation
Based on record review, the center failed to ensure that the separate record for each enrolled child contains all of the elements as required by the standards.
Evidence:
1. The record for Child 2 did not contain the following information: work phone number for each parent (an employer was listed but the record did not contain a work phone number) and name address and phone number of two designated people to call in an emergency if a parent cannot be reached (the addresses for both designated people were incomplete).
2. The record for Child 4 did not contain the following information: name address and phone number of two designated people to call in an emergency if a parent cannot be reached (the addresses for both designated people were incomplete).
Plan of Correction: Staff will obtain information from parents.
