Inspector Notes

An unannounced non-mandated monitoring inspection was conducted on 09/21/2023. There were 37 children, ages 2years- 10 years, 3 staff members providing direct care and supervision and various support staff present during the inspection. The children were observed during arrival to the program, teacher directed activities and self-directed activities. The inspector reviewed compliance in the areas of administration, staff qualifications and training, and background checks. A total of 5 children?s records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on observation and record review, the center failed to ensure that the required staff to child ratio shall be maintained for 3-year-olds up to school age eligible.

Evidence:
1. There were 17 children observed in the green group with 1 staff member. The youngest child was 3 years old. The required staff ratio for this group of children was 1:10. Two staff would be required.

2. There were 16 children observed in the orange group with 1 staff member. The youngest child was 4 years old. The required staff ratio for this group of children was 1:10. Two staff would be required.
Plan of Correction: Provider will ensure classrooms are in ratio.

Based on record review and interviews with staff, the center failed to ensure that staff records contain all of the elements as required by the standards.

Evidence: The record for Staff 3 documented a hire date of 09/07/2023. The record for Staff 1 did not contain the following: documentation that two or more references as to character and reputation as well as competency were checked before employment. Staff 1 confirmed this information was not available.
Plan of Correction: Provider will obtain information prior to hire.
