Inspector Notes

A subsidy health and safety inspection was conducted on 02/27/2024. There were 18 children, ages 2 years - 5 years, 5 staff members providing direct care and supervision present during the inspection and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies, and nutrition. The children were observed during child directed activities, mealtime, departing and arriving from a field trip. The inspector discussed the following with the provider: hazardous materials, allergy list and field trips. A total of 5 children?s records, 3 staff records, and 2 agent/applicant records were reviewed. The inspector arrived for the inspection at approximately 10:50 am and departed at approximately 1:15 pm.

A licensing inspection was also conducted on this date. Both inspections combined cover health and safety regulations. Only subsidy requirements not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review and interviews with staff, the vendor failed to ensure that all staff who work directly with children shall have with 90 days of the date of employment: current certification in CPR and current certification in first aid.

Evidence:
1. The most recent certification in CPR and First Aid for Staff 1 expired 01/2024. The hire date for Staff 1 is 11/27/2018.
2. The most recent certification in CPR and First Aid for Staff 2 expired 01/2024. The hire date for Staff 2 is 08/10/2020.
Plan of Correction: Training is scheduled.
