During the inspection, the inspector reviewed the areas listed above to. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 09/24/2025 and completed on 11/06/2025 in response to a complaint received by the licensing office on 09/16/2025 relating to allegations of staff qualifications and training, transportation, and staffing and supervision. The inspector reviewed 6 children?s records and 1 staff record onsite and electronically on 10/02/2025. This inspection included document review, and interviews.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/14/2025. A POC submitted after this date will not appear on the public website.

SYSTEMIC VIOLATION
The center is required to obtain a completed sworn statement prior to employment or permitting an individual to serve as a volunteer who is alone with, in control of, or supervising children.

There was no sworn statement for Volunteer 1 acting in the role of a staff member and required to meet standards for staff. Volunteer 1 was working alone with children and transporting children for seven working days.
Plan of Correction: The center will make sure the sworn statement is fill out with all the paperwork before any duties are performed.

REPEAT VIOLATION
The center must obtain a completed national criminal background check prior to employment or permitting an individual to serve as a volunteer who is alone with, in control of, or supervising children.

There was no criminal background check completed for Volunteer 1, acting in the role of a staff member and required to meet standards for staff. Volunteer 1 was working alone with children and transporting children for seven working days.

REPEAT VIOLATION
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children.

There was no documentation of a negative tuberculosis screening for Volunteer 1, acting in the role of a staff member and required to meet standards for staff. Volunteer 1 was working alone with children for seven working days and was in contact with children.
Plan of Correction: The center will ensure that all staff have a tuberculosis test on file before any tasks are performed.

REPEAT VIOLATION
Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

There was no documentation of orientation training for Volunteer 1, acting in the role of a staff member and required to meet standards for staff. Volunteer 1 was working alone with children for seven working days.
Plan of Correction: The center will make sure that all staff has all documentation completed and filed before performing any job responsibilities.

Prior to working alone with children and within seven days of the first day of employment, staff shall be provided in writing all of the required information.

There was no documentation of required information provided in writing to Volunteer 1, acting in the role of a staff member and required to meet standards for staff. Volunteer 1 was working alone with children for seven working days.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection and guidance.

For seven working days in September 2025, Volunteer 1, acting in the role of a staff member and required to meet standards for staff, was alone with and transported school age children enrolled in the center. Volunteer 1 did not have any required background checks, health documentation, center orientation training, and qualifications as a driver.
Plan of Correction: The center will make sure that all background checks are in and the staff member meets all qualifications before they perform any duties at the center.
Standard 8VAC20-780-550-K
The center shall prepare a document containing all the required emergency information that must be kept in vehicles used to transport children to and from center.

For seven working days in September 2025, the vehicle used to transport children by Volunteer 1 did not contain the required document.
Plan of Correction: The center will make sure that all emergency information will be put on all forms of transportation.

The center shall ensure that during transportation of children the following information is in the transportation vehicles: emergency numbers; and the center?s name, address and phone number.

The required information was not provided on the vehicle used by Volunteer 1 to transport children.
Plan of Correction: The center will make sure that all emergency information will be put on all forms of transportation.

Staff records shall be kept for each staff person that contain all of the required information.

There was no staff record for Volunteer 1, acting in the role of a staff member and required to meet standards for staff. Volunteer 1 was working alone with children for seven working days.
