During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 03/26/2026, as a part of probation. The Department issued a Special Order on 02/12/2026 to place the licensee on probation for a period of six months and prohibit new admissions of children for a period of 60 days as a result of high-risk violations. The on-site inspection began at 3:00 p.m. and ended at 3:45 p.m. The inspector reviewed compliance in the areas listed above. There were 41 children present and 5 staff. The inspector reviewed 1 children?s record and 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid.

Staff 1 was alone with a group of seven, three and four-year-old children and was not certified in CPR or first aid.
Plan of Correction: Staff is scheduled for certification training.
Standard 8VAC20-780-70
Documentation of at least two references to character, reputation, and competency are to be checked prior to employment.

There was no documentation of two references for Staff 1 (employed one month).
Plan of Correction: Provider will obtain references.
