The Licensing Inspector conducted an investigation on 12/15/2021, in response to a complaint that was received by the licensing office on 12/07/2021 regarding physical plant and supervision. Based on staff interviews, records reviewed, and observations made, the preponderance of evidence gathered during this investigation does not support the allegations.
