Inspector Notes

An unannounced renewal inspection was conducted today from 10:50am-12:15pm. There were 187 children (infants-5yrs.) directly supervised by 40 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed napping, participating in circle time, reading books, participating in group play, eating and playing on the playground. There was an adequate supply of materials for the children. Staff's MAT, CPR and First Aid certifications were verified. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Standard 8VAC20-780-130-E
The center did not obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
Child#3's record contains an immunization record that was last updated on 6/11/2021.
Plan of Correction: We have received the record via email 3/21/2022 on the inspection day. All records will be obtained according to the standard.
Standard 8VAC20-780-160-A-1
Based on record review, a staff's tuberculosis (TB) test/screening results were not obtained at the time of employment and prior to coming into contact with children.

Evidence:
Staff #2's (DOH: 3/14/2022) record contains a TB test/screening that was completed on 3/15/2022.
Plan of Correction: All staff members will obtain TB test prior to working. We will provide a referral for new staff to complete at Care Now and GPA will cover the cost.
Standard 8VAC20-780-270-A
Based on record review, the center did not maintained all areas of the facility in a safe condition.

Evidence:
In the PreK classroom, an area of approximately 1 inch by 3 inches of peeling paint was observed under the hand soap dispenser used by the children in care.
Plan of Correction: We have contacted our painter who will repair the wall. The soap dispenser was replaced last week 3/14/2022 and damaged the wall.

Based on observation, cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
Two bottles of disinfectant solution labeled "Keep out of reach of children" were observed in the facility's laundry room, the door to the laundry room was closed but it was not locked during the inspection.
Plan of Correction: We have installed a child safety lock on the door.
