Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/2/2025, as a part of the licensure period. The on-site inspection began at 12:35pm and ended at 3:14pm. The inspector reviewed compliance in the areas listed above. There were 166 children present and 48 staff. The inspector reviewed 8 children?s records and 5 staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.
Medication for child #2 was not labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.
Plan of Correction: Administration will obtain required medication labeled medication from parent(s) for current medication and all medications from now on.

Bottle fed infants who cannot hold their own bottles shall be held when fed.
An infant who could hold its own bottle was bottle fed while in on its back, not being held.
Plan of Correction: Corrected during inspection. Administrators will review bottle feeding protocols with staff.
