Inspector Notes

An unannounced renewal inspection was conducted today with the provider, who was present with 9 children and a second caregiver. The age of the children ranged from 11 months to 5 years for a total of 18 points. The children and provider were preparing for lunch at the conclusion of the inspection. Lunch consisted of pizza rolls, banana, carrots and milk.

Five children's files were reviewed today.

Based on a review of five children's files and an interview with the provider, it was determined that a immunization record was not obtained for one child before the child attended the family day home.
Evidence:
Child A enrolled on 2/14/22 and there was no immunization record on file.
Plan of Correction: I will get the immunization record.
