Inspector Notes

An unannounced renewal inspection was conducted with the provider, who was present with 12 children and one assistant.
The older children were on their mats watching tv, while the younger children were napping at the beginning of the inspection. The Licensing Inspector was able to see the transition to morning snack, which consisted of bagels, cream cheese, apple and juice.

There were no medication on stie for any of the children nor have their been any injury/accident situations requiring documentation.
The provider's First Aid and CPR are through American Safety Health Institute and will expire on 10/2/23.
Household pets, Kingley's rabies vaccination is dated 7/23/22 and Leo's rabies vaccination expires on 9/26/23.
Within 10 days send documentation to show that you have reported for fingerprint background checks, upon completion please send copies of the completed background checks.
The area of non compliance is outlned on the violation notice.

Based on a review of files and interview, it was determined that there were no repeat fingerprint background checks every five years thereafter.
Evidence:
Person A's background check was dated 3/5/18, Person B's background check was dated 3/6/18 and Person C's background check was dated 3/14/18. All three background checks had not been renewed within 5 years as required.
Plan of Correction: I will obtained these background checks.
