Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/24/2024 . The on-site inspection began at 10:45 and ended at 11:45. The inspector reviewed compliance in the areas listed above. There were 6 children present and 3 staff.
The inspector reviewed 5 children?s records on site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Centers children three years of age or yonder may not be offered foods that are considered potential choking hazards.

Tortilla chips were being served to children three years of age and younger during lunch.
Plan of Correction: Center staff have options to choose from the school menu and will choose non-choking hazard items. The health coordinator will review menus and assist in making sure choking hazards are not served to children three and under.

Child's records are required to contain documentation of viewing proof of the child's identity and age within 7 days of enrollment.

The record for child #3 was missing documentation of viewing proof of the child's identity an age. The child had been enrolled for over 1 month.
Plan of Correction: Staff have requested documentation from the parents of proof and ID.
