Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.
A monitoring inspection was initiated on 8/23/2021 and concluded on 8/26/2021. The Provider was contacted by telephone and a virtual inspection was conducted. There were 9 children present (26 points), ranging in ages from 9 months to 3 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 4 staff records, and 1 household member record were reviewed.

Based on a review of children's records, 1 out of 2 records did not have the required emergency contact information.

Evidence:
1. Child #2's record was missing the name, address and telephone number of one of two required emergency contacts.
Plan of Correction: I will ask the parents for the information for a second emergency contact today.
