Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on September 23, 2024. The on-site inspection began at 9:40am and ended at 11:00am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers; the point total was 12. The inspector reviewed 5 children's records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction {POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 30, 2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening.

The most recent screening for Household Member #2, was completed two years and 6 months ago.

Repeat Violation

Areas of the family day home, outside, shall be maintained in a clean, safe, and operable condition.

On the play structure, the large yellow tube slide was cracked in three places. The wood that attached the large yellow tube slide to the top of the play structure was un-attached at one of four points. The bottom step on the wooden ladder attached to the play structure, was cracked at the bottom exposing cracked and splintering wood.

In the backyard play area, a plastic wagon was cracked exposing sharp plastic.
Plan of Correction: The yellow tube will be removed and everything will get fixed on the weekend.

When meals or snacks are provided by the family day home, the menu for the current one-week period shall be dated.

The menu, was not dated and only contained meals and snacks for one day.
