Inspector Notes
An unannounced complaint inspection was conducted on site on 4/15/2022. The inspectors arrived at 9:15 AM and departed at 11:45A.M. Follow up interviews were completed on 5/20/2022. A complaint was received in the licensing office on 4/13/2022. The preponderance of evidence of sight and sound supervision was supported and complaint is valid.

Based on staff interviews, the center failed to ensure that all children were within sight and sound supervision at all times.
Evidence:
1. On April 11, 2022 child # 1 was left on the playground for approximately 5 minutes. Staff # 1 stated that all of the older children (ages 2-5) were lining up at the door and head counts were not completed. Normally they enter one class at a time and on this day a staff person opened the door and all three classes entered at one time. A parent entering the center saw the child on the playground.
The child was standing behind a brick wall that was six feet deep and three foot wide that allowed the child to hide without the staff being able to see the child.

Plan of Correction: A new Early Child Hood Director was hired. New polices and procedures are in place to help ensure that all children are accounted for at all times. The new director has implemented a new accounting system that uses lanyards to have a record of each child in the classroom.
