During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 09/02/2025. The on-site inspection began at 10:15a.m. and ended at 1:15 p.m. The inspector reviewed compliance in the areas listed above. There were 39 children present and 13 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-500-A
Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine. Staff shall wash hands with soap and running water before and after a diaper change. Staff shall wash hands with soap and running water before and after a diaper change.

Staff 6 changed a child?s diaper and wiped the child?s hands with a disposable wipe and did not wash the child's hands with soap and running water. Staff 6 used hand sanitizer and not soap and running water after changing a child?s diaper.
Plan of Correction: Staff will follow the requirements regarding handwashing. .
Standard 8VAC20-780-500-B
The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use.

Staff 6 sanitized the diaper changing surface but did not clean the surface with soap and at least room temperature water.
Plan of Correction: Staff will follow the requirements regarding cleaning and sanitizing.
Standard 8VAC20-780-60-A-8
A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child 1, who has been enrolled for three weeks, has a diagnosed food allergy and did not have a written care plan in their file.
