Standard 8VAC20-780-40-E
The operational responsibilities of the license shall include ensuring that the activities, services and facilities are maintained in compliance with the center?s own policies and procedures.

The center?s injury prevention plan states that ?staff will immediately complete an incident report for all accidents/injuries.? On 10/23/2025 an injury occurred to Child 1, and no report was completed.
Plan of Correction: Part-time teacher submitted an incident report on 10/28/2025. All staff have been reminded that an incident report must be logged on Brightwheel in a timely manner.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children?s serious and minor injuries in which entries are made the date of occurrence.

Child 1 (16 months old) sustained an injury while under the supervision of the center on 10/23/2025. There was not a written record of the injury.
Plan of Correction: Part-time teacher submitted an incident report on 10/28/2025. All staff have been reminded that an incident report must be logged on Brightwheel in a timely manner.
Standard 8VAC20-780-80-B
The center shall inform the department?s representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

Child 1 (16 months old) sustained an injury, while under the supervision of the center on 10/23/2025, that required outside medical attention. The center did not notify the department that the injury occurred.
Plan of Correction: During the inspection, a report was submitted detailing the incident that required outside medical attention. The expectation of such reports were reviewed with the inspector. Moving forward the director will ensure that such reports are made in a timely manner.
