During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or
fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 04/09/2025, as a part of the licensure period. The on-site inspection began at 11:10a.m. and ended at 12:45p.m. The inspector reviewed compliance in the areas listed above. There were 9 children present and 2 caregivers; the point total was 17. The inspector reviewed 5 children?s records and 3 caregiver's records and 1 household member's record on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Documentation of the orientation shall be signed and dated by the provider and assistant.

Orientation training for Staff #2 was not signed and dated by the provider and the staff.
Plan of Correction: The orientation training will be signed by both the provider and this staff.
Standard 8VAC20-800-60-B
Children's records are required to contain a start date and medical insurance policy and number.

Child #2's record was missing the insurance policy number. Child #3's record was missing the name of the insurance policy and the policy number.

Child#5's record did not contain the child's first date of attendance.
Plan of Correction: The insurance information will be obtained and documented in their records. Start date for the one child will be documented.

Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized.

Child # 3 had been attending the family day home for 10 days before the provider obtained documentation that this child was adequately immunized.
Plan of Correction: Ongoing, a child will not begin care until immunization documentation is obtained.
