Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on and 4/15/2021 concluded on 4/15/2021. The provider was contacted by telephone to initiate the inspection. There were 3 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records, 1 household member record and 1 staff record submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Child#1's record did not contain the parents' work phone numbers and an address for 1 of 2 emergency contacts.
Evidence:
The medication authorization form for a prescription allergy medication for Child#3 expired on 9/30/2020. The medication was observed on-site during the inspection.
