Inspector Notes
An unannounced monitoring inspection was conducted today from 2:20pm-2:50pm. There were 2 children (6 points) directly supervised by the provider. The home, 1 staff record, 2 children?s records, 3 household member?s records, evacuation drills, emergency supplies and policies were inspected. Children were observed napping and interacting with the provider. There was an adequate supply of books, toys, and materials for the children. The home was clean and organized. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

3Evidence:
The immunization records for Child #1 and Child #2 were not stamped or signed by the licensed physician.
Based on record review, the provider did not obtain a Report of Tuberculosis Screening form for a household member.
Evidence:
HH#2's record did not contain a Report of Tuberculosis Screening form.
Based on record review, emergency evacuation procedures were not practiced monthly.
Evidence:
Evacuation drills were not practiced in October 2023.
