During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 11/14/2024, as part of the licensure period. The on-site inspection began at 12:45pm and ended at 1:50pm. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver; the point total was 8. The inspector reviewed 3 children?s records and 4 caregiver/household members' records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening.

An updated TB screening for Household Member #1 was not completed and is overdue by three months.
Plan of Correction: Provider will have household member schedule TB screening today.
Standard 8VAC20-800-340-C
The bathroom shall contain liquid soap.

The family day home bathroom did not contain liquid soap.
Plan of Correction: Provider will purchase and have available liquid hand soap in the bathroom.
Standard 8VAC20-800-50-A
The family day home shall keep a written record of children in attendance each day.

There were no attendance records on the date of inspection. The provider did not take written attendance on November 11/12-11/14, 2024.
Plan of Correction: Provider will document daily attendance.
Standard 8VAC20-800-60-B
A child's record shall contain name and policy number of the child's medical insurance.

Child #1's record did not contain insurance policy number.
Plan of Correction: Provider will request policy number at pick up.

The emergency evacuation procedures shall be practiced monthly.
