Inspector Notes

A monitoring inspection was conducted on October 16, 2023 from 5:15pm to approximately 6:40pm. There were 14 children present and three staff members. The Inspector reviewed four children?s records and three staff records. The Inspector observed the interaction of staff and children throughout the inspection. Children were observed during group time and free play prior to being picked up by their parents. First aid kits and emergency preparedness documents were observed. Medications were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program. The violation was discussed with the program manager over the phone.

Based on record review and interview, it was determined that the center did not ensure that at least every two years from the date of the initial screening or testing, or more frequently if recommended by a licensed physician or the local health department, staff members shall obtain and submit the results of a follow-up tuberculosis (TB) screening.
Evidence:
1. Staff #1 has a TB screening dated 8/25/2019, which is expired and not within two years from the date of the inspection and date of their initial screening.
2. Staff #2 has a TB screening dated 11/08/2020, which is expired and not within two years from the date of the inspection and date of their initial screening.
Plan of Correction: The center responded with the following: The center will obtain new and recent documentation of a negative tuberculosis screenings for Staff #1 and Staff #2.
